How to Finish Millicent's Quest in Elden Ring and Get the Best Rewards
Millicent’s questline stands as one of the most emotionally resonant and mechanically rewarding journeys in the Lands Between. Spanning from the desolate rot-lands of Caelid to the heights of the Haligtree, this path offers essential items for specific character builds and a crucial tool for those seeking to alter the game's ending. Completing this odyssey requires patience, as it runs parallel to the main story progression through several endgame regions.
Starting the Journey in Caelid
The story begins at Gowry’s Shack, located south of Sellia, Town of Sorcery. You will encounter Sage Gowry, who requests your assistance in healing a girl succumbing to scarlet rot.
To proceed, you must venture into the Swamp of Aeonia to the east. Here, you face Commander O'Neil. Defeating him yields the Unalloyed Gold Needle. This boss can be challenging due to his summons; utilizing Torrent for hit-and-run tactics is a viable approach. After obtaining the needle, return it to Gowry. He will ask for time to repair it. Resting at a Site of Grace or reloading the area will prompt him to give you the repaired needle along with the secret of Sellia.
Healing Millicent at the Church of the Plague
Take the repaired needle to the Church of the Plague, situated on the cliffs above Sellia. You can reach this by solving the candle puzzle in the town or by dropping down from the Dragonbarrow heights to the north. Give the needle to Millicent, who is resting inside the church. After she uses it, exhaust her dialogue and rest at the nearby grace. She will grant you the Prosthesis-Wearer Heirloom, a valuable talisman for Dexterity-based builds.
Millicent will eventually move to Gowry’s Shack once more. Speak with her there before she sets off on her own journey to retrace the steps of Malenia.
Finding the Valkyrie’s Prosthesis
Your next encounter with Millicent occurs in the Altus Plateau, specifically near the Erdtree-Gazing Hill. She laments her inability to wield a sword effectively with only one arm. To help her, you must travel to the Shaded Castle in the northern valley of Altus.
Inside the castle, look for a chest in a room guarded by a Cleanrot Knight on the northwestern side. This chest contains the Valkyrie’s Prosthesis. Return this item to Millicent at Erdtree-Gazing Hill. Giving her the arm allows her to be summoned for various boss fights, including the Godskin Apostle at Windmill Village.
The Path to the Haligtree
After defeating the Godskin Apostle in Dominula, Windmill Village, Millicent appears near the Windmill Heights Site of Grace. Ensure you speak with her until her dialogue is exhausted. Her journey then continues to the Mountaintops of the Giants, specifically at the Ancient Snow Valley Ruins grace.
From there, the final stages of her quest take place within Elphael, Brace of the Haligtree. This secret area requires the Haligtree Secret Medallion to access. You will find her at the Prayer Room Site of Grace. She shares more about her connection to Malenia, suggesting she may be a sister or an offshoot of the demigod.
The Final Choice: Assistance or Betrayal
The climax of Millicent’s quest occurs after defeating the Ulcerated Tree Spirit in the scarlet rot pond near the Drainage Channel Site of Grace. After the beast is dead, two summon signs will appear on a nearby slope:
- The Gold Sign (Assist Millicent): You join Millicent to fight her four sisters. Success rewards you with the Rotten Winged Sword Insignia, which significantly boosts attack power with successive hits. After the battle, Millicent will be found nearby. Speak to her, reload the area, and you can retrieve the Unalloyed Gold Needle from her remains. This needle is essential for those who wish to reverse the Lord of Frenzied Flame ending by using it in Malenia’s boss arena.
- The Red Sign (Challenge Millicent): You invade and kill Millicent. This rewards you with Millicent’s Prosthesis, a talisman that boosts Dexterity and increases attack power with successive hits. While this provides a unique item, it prevents you from obtaining the needle used to cure the Frenzied Flame.
Choosing between these rewards often depends on your build. The Rotten Winged Sword Insignia is generally considered superior for fast-attacking weapons, while Millicent's Prosthesis is a solid choice for reaching Dexterity caps.
